Make a Template for a CinC Paper

If your paper has been accepted, please refer to your acceptance letter for its control number.

If you plan to submit a paper for pre-acceptance review, submit a conference program abstract of it first. You will receive a control number as the final step in doing so, once you mark your abstract as ready for review.

Once you know your control number, use this form to make a LaTeX or MS Word template with a properly formatted title block for your CinC paper. (The MS Word template can be used with Open Office.) The resulting template will be e-mailed to the address that you supply below.

Title: Please enter the title of your paper, using Mixed Capitals. Avoid using abbreviations and acronyms, such as ECG or MRI. Example: The Use of Magnetic Resonance Imaging in the Detection of Mitral Valve Stenosis

Organizations: Please enter the affiliations of all authors, one on each line. Avoid using abbreviations other than UK, USA, and the standard two-letter abbreviations for names of U.S. states. Do not include street addresses or postal codes; you should include a full postal address for correspondence at the end of your paper, but not here. Example: University of Podunk, Podunk, IL, USA

Authors: Please enter the name of each author, in the order that you want them to be listed. For each author, please enter the affiliation number in the box to the left of the author's name. If any author has more than one affiliation, please enter the numbers separated by commas (example: 1,2). Enter the initials of the given name(s) author in capital letters and with no punctuation.
